APPIC 2012 Conference Registration Form



Before you Register

Welcome to APPIC's Online Registration Page for the 2012 Conference to be held in Tempe, Arizona, April 26-28. Hotel Rooms at the Tempe Mission Palms: Click Here. We are excited about our programming and looking forward to being your host. Here are a few things to know prior to registering for the conference:

  1. The Conference fee for APPIC Members is $345.00 before March 15, 2012. (Early Registration Rate)
  2. NonMembers may register for $400 before March 15, 2012.
  3. Directors of Clinical Training at APPIC Doctoral Program Associates (DPAs) also can register for $345 before March 15, 2012.
  4. AFTER March 15, 2012 all registrations are $425.00. (Later Registration Rate)
  5. The conference fee includes the Thursday evening reception; a full breakfast and lunch on Friday and Saturday; snack/beverage breaks both morning and afternoon on Friday and Saturday; CE credits; transportation to and from the Phoenix airport (for those staying at the hotel and using the shuttle bus available 5:30am-10:30pm); audio-visual package for the conference that includes wireless access in the large meeting room and your hotel room, and the poster presentation and reception on Friday afternoon (light appetizers/cash bar).
  6. If you plan to use a credit card to register - have it ready. We ONLY accept Master Card and VISA. You may also pay by check.
  7. You must fully complete the registration form before saving since the online system does not save an incomplete registration form.
  8. If you are planning to pay by check, you may do so. When we receive your check in the mail, we will email you at the email address on the registration form. Your registration will be complete at that time. Your check should be made out to: APPIC and mailed to: 17225 El Camino Real - Suite #170 - Houston, Tx 77058. Please make sure a copy of the registration form accompanies your check.
  9. Please "print and save" your registration form for your records if needed.
  10. The name used on the registration form will be the name that appears on your badge.
  11. There is no additional cost to attend the APA CoA accreditation workshops BUT you must register with APA Office of Program Consultation & Accreditation (202) 336-5979 and register for the APPIC Conference.

Cancellation/Refund Policy:
